In September, a woman with dual diagnoses and who had recently lost her son to suicide became a member of MI Health Link. The member experienced chronic mental and physical health conditions exacerbated by uncoordinated care.
Upon enrollment, she connected with a primary care physician and also was provided grief counseling.
With another member who had suffered from depression and grief, she was able to develop a “life plan.” The plan includes a crisis plan with identified supports for times when suicide is considered, the development of goals, interventions, and objectives, which include developing and maintaining supportive relationships to maintain a stable mood and improved health.
The care coordinator has developed a close relationship with her and is assisting her during this difficult time in her life and assuring good coordination of care overall.
